Our mission is to improve the health of our community by delivering high-quality, affordable healthcare through culturally competent and linguistically appropriate services.
At Chinese Community Health Plan (CCHP), we offer health, wellness and preventive coverage to individuals, families, and employers of all sizes, and Medicare Advantage Prescription Drug plans in San Francisco and northern San Mateo counties.
As of June 2014, CCHP enrollments were 28,500 active members including 8,800 in Medicare Advantage programs and 19,700 in Individuals, Families, Covered California and Employer Group plans. The make-up of our multicultural membership consists of 65% Chinese and 35% non-Chinese, latter of which is made-up of Caucasian, African-American, Hispanic and other Asians ethnicities.
Award-Winning Health Programs
In 2014, CCHP received a Bronze Award for the prevention of heart attacks, strokes and diabetic complications by California Department of Managed Health Care (DMHC).
In 2010, in conjunction with Chinese Hospital, awarded the National Council of Asian Pacific Islander Physicians (NCAPIP) Inaugural Excellence in Health Award for being pioneers in providing access to quality and affordable care for the Chinese American community in San Francisco for over one hundred years.
Winner of the 2007 National Committee for Quality Assurance (NCQA) “Recognizing Innovation in Multicultural Health Care Award” for our diabetes self-management program to improve diabetes care in the Chinese community
Winner of the 2006 National Committee for Quality Assurance (NCQA) “Recognizing Innovation in Multicultural Health Care Award” for our efforts to reach across cultural divides and provide enrollees in San Francisco with culturally and linguistically appropriate health care services
Awarded the 2000 American Association of Health Plans “Ellis J. Bonner Community Leadership Award” in recognition of the distinguished contribution that CCHP made to enhance the quality of life to the Chinese community
