Privacy and Disclosure Statements

CCHP makes the following assurances of privacy for visitors to the website:

  • The website is designed to provide useful information about the CCHP organization and about health.
  • The website does not contain any advertising for products by third parties, nor does CCHP share any of your information with other parties except as authorized by law.
  • CCHP tracks visits to the website via an automatic monitoring program that records, among other things: how many visits are made to the website; the time of day and date of those visits; and the areas visited. This information is used to evaluate the effectiveness of the website. Who Funds the Website

The website is funded entirely by the operating income of the organization. CCHP does not accept external advertising income in exchange for the promotion of any product or service via the website.

The website may have numerous links to other Internet pages maintained by third parties, which may have information on health topics of interest to you. CCHP, however, does not operate, control, sponsor or endorse any of these websites, nor does CCHP make any representations regarding their content or accuracy. Liability of CCHP

Jurisdiction

You expressly agree that exclusive jurisdiction for any dispute with CCHP, or in any way relating to your use of the website, resides in the courts of the State of California and you further agree and expressly consent to the exercise of personal jurisdiction in the courts of the State of California in connection with any such dispute including any claim involving CCHP or its affiliates, subsidiaries, employees, contractors, officers, directors, telecommunication providers, and content providers.

The terms and conditions of use are governed by the internal substantive laws of the State of California, without respect to its conflict of laws principles. If any provision of the terms and conditions of use are found to be invalid by any court having competent jurisdiction, the invalidity of such provision shall not affect the validity of the remaining provisions of the terms and conditions of use, which shall remain in full force and effect. No waiver of any of the terms and conditions of use shall be deemed a further or continuing waiver of such term or condition of use or any other terms or conditions.

Chinese Community Health Plan (CCHP) is a Medicare Advantage HMO plan with a Medicare contract and a California Medicaid program contract for our SNP. Enrollment in CCHP depends on contract renewal. Every year, Medicare evaluates plans based on a 5-star rating system. CCHP’s pharmacy network offers limited access to pharmacies with preferred cost-sharing in San Francisco and San Mateo County. The lower costs advertised in our plan materials for these pharmacies may not be available at the pharmacy you use.
